Enrollment Applications and Claim Forms

Please note that all Forms on this Website that require a signature (from Plan Member; Physician; Employer)
can be completed on-line but must be printed, signed and dated prior to mailing to A.R.M. Management Ltd.

Medical Services Plan of BC Enrollment Application

	The Medical Services Plan of BC Enrollment "Application for Group Entitlement" must be fully completed and submitted to A.R.M. Management Ltd., so that the Plan Administrator can commence your MSP of BC coverage once you have established your Health Care Plan eligibility.

Premium Assistance Application for the MSP of BC

	The Medical Services Plan of BC coverage is a taxable benefit and as such the Plan Trustees encourage all members to complete and submit a Premium Assistance Application along with their MSP of BC enrollment application so that the member may take advantage of the premium assistance offered by the Provincial Government thereby reducing the income tax the member will pay each year.

Health Care Plan Enrollment and Beneficiary Designation Form

	The Health Care Plan Enrollment Application and Beneficiary Designation Form must be completed by all Plan members and submitted to A.R.M. Management Ltd., in order to provide the Plan Administrator with the personal information necessary to enroll you, and where applicable your dependents, in the Health Care Plan.

Change of Beneficiary and/or Addition of a New Dependent

This Change of Beneficiary Form should be fully completed and submitted to A.R.M. Management Ltd., to change your Health Care Plan Beneficiary.

MSP of BC Group Change Form (Add/Remove a dependent to or from your MSP of BC coverage)

This MSP of BC Group Change Form should be fully completed and submitted to A.R.M. Management Ltd., when you wish to add or remove a dependent from your Medical Services Plan of BC coverage.

Extended Health Care Benefit Claim Form

This Extended Health Care Benefit claim form should be completed and submitted to A.R.M. Management Ltd. Please include the original receipts of the item(s) you are submitting for reimbursement from the Health Care Plan.
