Greater Vancouver Hotel Employers' Association - Local 40 Health Care PlanMedical Services Plan of British Columbia:This coverage is available to all who qualify but as this is a taxable benefit members already covered under the Medical Services Plan of B.C. through their spouse or family, need not take out duplicate Medical Services Plan of B.C. coverage. The medical coverage is not required for Dental and/or other Health Care Plan Benefits. Eligibility for Medical Services Plan of B.C. coverage is determined by the Medical Services Plan of B.C. You will be required to provide proof of Canadian citizenship or up-to-date immigration documents for yourself and each of your family members. You will be required to provide on-going proof of an overage dependent's attendance at school. If your total annual family income is less than $28,000 you may be eligible for monthly premium reduction. An Application for this premium reduction is available from this Website or from the Office of the Plan Administrator.The Medical Services Plan of B.C. is mandatory for all residents of British Columbia so please be certain to apply for this coverage should you not be eligible for the coverage as a dependent with your spouse or family. Accident & Sickness (Wage Loss) Benefit:The Accident & Sickness (wage Loss) Benefit is included in the GVHEA - Local 40 Health Care Plan. Please refer to your Employer's Benefit Summary additional information. Dental Care Benefit:The Dental Care Benefit is included in the GVHEA - Local 40 Health Care Plan. Please refer to your Employer's Benefit Summary for additional information. Extended Health Care Benefit:The Extended Health Care Benefit is included in the GVHEA - Local 40 Health Care Plan. Please refer to your Employer's Benefit Summary for additional information. All Health Care Plan participants must register with the British Columbia Fair Pharmacare Program and provide proof of registration to the Office of the Administrator. Failure to register with the British Columbia Fair Pharmacare Program will result in declination of the plan participant and their dependents prescription drugs claims incurred prior to their BC Fair Pharmacare Registration date as per instruction from the Health Care Plan Board of Trustees. Vision Care Benefit:The Vision Care Benefit is included in the GVHEA - Local 40 Health Care Plan. Please refer to your Employer's Benefit Summary for additional information. Life Insurance Benefit:The Life Insurance Benefit is included in the GVHEA - Local 40 Health Care Plan. Please refer to your Employer's Benefit Summary for additional information. Group Life Insurance Conversion Privilege:The Canadian Life and Health Insurance Association (CLHIA) has established
guidelines that describe the conditions under which a group Health Care Plan
member may continue their Life Insurance coverage on their own when their group
life insurance ends. Accidental Death & Dismemberment Benefit:The Accidental Death & Dismemberment Benefit is included in the GVHEA - Local 40 Health Care Plan.
